How is this different from a normal back belt?

A standard belt compresses your waist and stops there. This has two bands you tighten separately: a lower one that wraps the pelvis and hips, and an upper one across the lower back. The pelvis is where the whole chain rests, which is why leaving it out never quite worked.

How do I pick my size?

Can I wear it under clothes?
